Project H.E.A.L. (free care)
Project H.E.A.L. (Health, Education, Access, Link) is a health care outreach program funded through CentraCare Foundation. This vounteer-led program offers free health screenings and basic care across Central Minnesota for residents with little or no health insurance. Project H.E.A.L. currently offers free monthly clinics and wellness events at multiple locations.
• The Place of Hope shelter in St. Cloud
• The Lincoln Center shelter in St. Cloud
• Archangel Recovery Center in St. Cloud
• Health events at the Islamic Center in St. Cloud
• Long Prairie clinic that is geared toward the Amish Community
• Melrose Senior Community Center geared toward the Hispanic community
How donations helped in 2024
• Over 500 individuals received care at Project H.E.A.L. clinics and wellness events.
• 82 vaccines were administered at Project H.E.A.L. clinics.
• 52 Project H.E.A.L. patients were successfully connected to primary care appointments in 2024.
CentraCare - Coordinated Care Clinic
CentraCare – Coordinated Care Clinic is designed for patients with complicated medical, mental health and social needs that can result in frequent hospitalization. The clinic provides additional resources and support to Project H.E.A.L. to ensure community members with healthcare needs are seen as soon as possible. Care options include:
• Primary care
• Mental health
• Office-based opioid treatment
• Medication therapy management
• Outreach services
• Social and community resource management
